UK – PRESTIGE-AF Patient Information Sheets

The box on the right allows you to download the information sheets and consent forms for the four different studies that are being undertaken as part of PRESTIGE-AF. One is the main PRESTIGE-AF study, whilst the other three are sub-studies that you may be asked to consider enrolling in if you are taking part in the main study. Your clinician will tell you which of the studies they are recommending you to consider enrolling in.

The information sheet will give you all the information about the study, why the research is taking place, and what it will involve so that you can make the decision that’s right for you.

There are three different types of information sheet, depending on the situation the patient that is considering enrolling in the study is in, be it yourself or a patient you are representing.

They are as follows:

  • Maintained capacity: for patients who are able to make the decision for themselves
  • Lack of capacity: for a relative, partner or close friend of a patient acting on their behalf because they, at this current time, are unable to make a decision for themselves
  • Regained capacity: for patients who were enrolled in a study by a relative, partner or close friend during a time when they were not well and were unable to make the decision for themselves, but are now able to make a decision about whether to continue taking part
